The State of Massachusetts requires that students attending college in the Commonwealth submit the required vaccinations and provide proper documentation. All forms must be completed, uploaded and entered into the Health Services Portal (see instructions below).

The deadline for submission is Monday, June 26, 2023 for Fall enrollment (January 1st/Spring enrollment). If all forms have not been uploaded and entered into the Health Services Portal within 30 days before the start of classes you will not be able to register for the following semester classes and may not be able to move onto campus. 

Health Services does not require a physical exam, but the "Immunization Requirements" portion of the form should be signed by your healthcare provider. 

The Emmanuel College Immunization & Health Form details which vaccines are required by the State of Massachusetts and those that are highly recommended by Health Services. Please note that for the ‘Immunization Requirements’ portion of the form you can substitute an official record from your healthcare provider. Please make sure that your documentation includes all of the required vaccines listed or positive titers where applicable as well as the completed Health History and TB Questionnaire. If you have not received all of the required vaccines you will need to obtain them prior to coming to campus. Health Services does not stock the state-required vaccines. 

Also, please upload copies of both the front and back of your medical insurance card. We suggest students keep a picture of their insurance card in their phone. 


  1. Take a picture of or scan each section of your forms and save them on your computer or phone. Do not use special characters when naming your files.

  2. Navigate to the EC Health Services Portal ( and sign in using your EC username and password. The student health portal can be accessed on your phone and your images can be uploaded directly to your portal profile using your camera. Please note that your images may not exceed 10 MB.

  3. Select "Document Upload" and upload the documents INDIVIDUALLY to their corresponding line in the dropdown menu. Once completed please do not send your forms to us, but instead maintain them for your records in case there is a problem with the image quality and you need to resubmit them. Please note that it may take up to 48 business hours to fully process your form.

The Commonwealth of MA requires all students enrolled in at least 3/4 time to be enrolled in a health insurance plan. More information on this requirement can be found here.

The following immunizations are required by Massachusetts law for all full-time (9 or more credits) entering undergraduate students including transfer students.

  • Emmanuel no longer requires the COVID-19 vaccine for new students. The College still strongly recommends that students be vaccinated against COVID-19 and receive boosters when eligible.
  • Please note that a physical exam is not required unless you are an athlete. For more information, refer to paperwork from the athletic department.
  • 2 MMR (measles, mumps, rubella) vaccines, the first given no earlier than one year after birth and the second at least 1 month after the first, or serology tests (titers) for measles, mumps and rubella that demonstrate immunity
  • 1 Tdap (tetanus, diptheria, pertussis) booster within the past 10 years.
  • Three hepatitis B vaccines or a hepatitis B serology test (titer) that demonstrates immunity.
  • Two varicella (chicken pox) vaccines or a history of varicella verified by your health care provider or a varicella titer that demonstrates immunity
  • A meningitis vaccine or a signed meningitis waiver. Please note that it is necessary to have either the vaccine or a signed waiver in order to receive your room key upon arrival. Signing the waiver as allowed by state regulation does not prevent you from receiving the vaccine at a later date, and in fact, we recommend that you do so.

The Massachusetts Department of Public Health strongly suggests TB testing (PPD) for students who are at increased risk for exposure to tuberculosis. Please review the TB Risk Questionnaire. If you have answered "yes" to any of the questions please bring the form to your health care provider for review.
