Important Forms

Entrance Health Form 

Who: All new students.
What: This health form, completed and signed by your doctor indicating you have had a physical exam within 12 months, is required of all new Emmanuel students. We suggest you keep a copy of the health form for your personal records.
When: All students must have this form completed before July 1, 2015. Any student who plans on playing a varsity sport at Emmanuel must have this form completed by July 1st, or she/he will not be able to participate in the sport.

Health Insurance Waiver/Enrollment Form

Who: All students.
What: The Commonwealth of Massachusetts requires all students enrolled at least 3/4 time to be enrolled in a health insurance plan. To ensure that Emmanuel College students are meeting this requirement, all students are automatically enrolled in the College's health plan and are charged the annual health insurance premium, which for 2014-2015 was $2,083.
If you have comparable coverage provided by a United States health insurance provider, you may waive enrollment in this plan by completing a waiver at www.universityhealthplans.com. The deadline to complete the waiver to avoid incurring the cost of the premium is August 5th. The 2015-2016 premium and online waiver will be available June 2015.

The 2015-2016 Free Application for Federal Student Aid (FAFSA)

Who: All students who wish to be considered for need-based financial aid from Emmanuel College.
What: The FAFSA is a free, federal form available at www.fafsa.gov that students must complete to be considered for need-based financial aid. Emmanuel's school code is 002147.
When: The 2015-2016 FAFSA becomes available on January 1, 2015. Emmanuel's priority filing date is February 15, 2015. If you apply for financial aid by the priority filing date, you should expect to receive an estimated financial aid award from the Office of Student Financial Services in March.

Printable Enrollment Form and Online Enrollment Deposit 

Who: You!
What: To let Emmanuel College know you'll be a member of the class entering in 2015, submit your enrollment form and enrollment deposit ($500 for resident students or $300 for commuter students) online or by mail to the Office of Admissions.
When: On or by May 1, 2015

I-20 Form 

Who: International students enrolling at Emmanuel College for the Fall 2015 semester.
What: To study on an F-1 visa in the United States at Emmanuel College, you must receive an I-20 from Emmanuel. You must submit your enrollment form and enrollment deposit before completing and submitting the I-20 form.
When: As soon as possible after submitting your enrollment form and enrollment deposit.

Residence Life + Housing Lease and Preferences Questionnaire 

Who: Any student planning to live on campus in the fall.
What: Students living on campus must complete the Residence Life + Housing Lease and Preferences Questionnaire through MySaints, our internal communication portal, which will be accessible after you send in your enrollment form and enrollment deposit. This document should be submitted as soon as possible so you can be matched with an Emmanuel roommate. Watch your Emmanuel e-mail for more information. If you have sent in your deposit and haven't yet created your Emmanuel e-mail, click here to create your student account.
