Health Insurance
All full-time (35 hours) employees are offered health insurance through Harvard Pilgrim Health Care.
www.harvardpilgrim.org
HMO
Individual Coverage: $114.90 per month
Family Coverage: $452.08 per month
What is an HMO? An HMO contracts with a number of health care providers - its "network." HMO members pay a set fee each year for a predefined package of services. Members must use providers in the "network" to obtain free (or fully reimbursed) care. HMO care requires members to chose a Primary Care Physician (PCP), who acts as a "gatekeeper" to coordinate members' care. Approval of the PCP is required before a member can see a specialist.
POS
Individual Coverage: $183.73 per month
Family Coverage: $632.59 per month
What is a Point of Service Plan? A POS plan combines some features of a typical fee-for-service plan with those of a prepaid managed care arrangement. These plans allow you to use in-network or out-of-network providers. Typically, the plan will cover all (or most) of the cost of in-network providers, while those using out-of-network services will incur co-pay and/or deductible out-of-pocket expenses.
Dental Insurance
Delta Dental
Individual Coverage: $13.98 per month
Family Coverage: $41.08 per month

Flexible Spending Account
All full-time (35 hours) employees can set aside money on a pre-tax/payroll deduction basis for IRS-approved unreimbursed medical, dental and day care expenses. Expenses such as office visits co-payments, laser eye surgery, prescription drug co-payments and deductibles which are typically paid by employees, can now be submitted for reimbursement with pre-tax dollars. The annual maximum allowed to be set aside pre-tax for medical expenses is $2,500. Employees will also be allowed to set aside up to $5,000 in a pre-tax dependent care account for day care expenses.
