- social security or school ID number
- day time phone number
- course name
- course number
We encourage you to register for classes early in the registration period and to register online! Students can obtain their registration forms in the Office of Graduate Studies.
You can talk with your assigned advisor to select courses for the forthcoming semester. If faculty consent is required, be sure to meet with the appropriate faculty member for his or her signature. If the courses you choose are filled by the time your form is processed, the alternate courses you list on the form will be entered. Closed courses will be posted in the Center for Graduate Studies and on the bulletin board outside of the Registrar's Office.
New students should complete the entire top portion of the registration form. Students registered in the prior semester will receive a pre-printed registration form. Please review the information printed and make any necessary corrections directly on the form. Changes to your major or minor must be made through the Center for Graduate Studies.
